Denial appeal
A draft appeal written in the payer's language.
Requesting records is one of those tasks that takes five minutes and somehow survives on the to-do list for three weeks. Your Assistant drafts the request with the details you provide.

Records request letter for a therapy client, drafted
Draft a records request to this client's previous provider using the contact information and authorization details already on file. Specify the records we need, keep the request appropriately scoped, and leave placeholders for anything missing.

Draft records request:
Please provide records relevant to Alex's prior outpatient behavioral health treatment, including the most recent diagnostic assessment, treatment summary, medication history if maintained by your practice, and discharge or transfer recommendations. A valid authorization for release is on file.
Fictional example · No real client data.
Name the prior provider, date range, and information you are requesting.
Get clear, professional language with the relevant client and authorization details left for review.
Confirm consent, destination, and any required forms before the request goes out.
Get the client's signed release of information, then send a written request to the provider naming the client, the records and date range you need, the purpose, where to send them and by when. Follow up if you hear nothing in two to three weeks.
The client's full name and date of birth, the records requested and date range, the purpose, a copy of the signed authorization, your practice details and secure delivery instructions, and a requested response date.
The request letter, from the details you provide, ready to send. It is a draft for your review; the release of information and the sending are yours.
Follow up in writing after two to three weeks, referencing the original request and its date. Give the Assistant the first letter and it drafts the follow-up.
