What does Alignment check compare?

The client's presenting concerns, treatment goals and their own words about what matters, against what has actually happened in recent sessions. It shows where the work is aligned, where priorities have shifted, and where a goal has fallen off the map.

Is this the same as a treatment plan review?

Related but narrower. A plan review asks whether the plan still fits. Alignment check asks whether the sessions are serving what the client came for, which sometimes reveals drift before the plan itself looks wrong.

When should I run it?

Before a treatment plan review, when a case feels like it has become about the thing currently on fire, or every few sessions as a habit. It takes a minute and the output is short.

What if the client's goals have changed?

Then that is the finding. It shows where the sessions have moved away from the original goals, and you decide whether the goals should catch up with the work or the work should return to the goals. Update treatment goals is the next job for that.
