Goals are the broad changes the client wants, such as fewer panic attacks. Objectives are the measurable steps toward them, with a target and a timeframe, such as using a grounding skill during three episodes this month. Interventions are what you do to get there.

How often should a treatment plan be updated?

Whenever something in the room changes direction, and at least at each formal review, typically every 90 days. Waiting for the review date when the client said something today that changes the plan is how plans go stale.
