Draft progress note:
Client reported increased work-related anxiety and avoidance of email following feedback from their supervisor. Therapist explored feared outcomes, identified an all-or-nothing prediction, and rehearsed a brief assertive response. Client generated an alternative statement and reported feeling more prepared to address the issue. Plan is to attempt the conversation before next session and review the outcome.

Whichever your payer, agency or group practice expects. SOAP separates subjective and objective data; DAP is shorter and common in private practice; BIRP foregrounds behavior and intervention. The content requirements are similar; the structure differs.
