There’s a reason I stepped back from clinical work.

It wasn't because I stopped believing in therapy — quite the opposite. It was because what I loved about the work became harder and harder to access, especially with the heavy administrative burden therapists face.

For example, the "golden thread" — that seamless connection between assessment, treatment planning, and progress tracking that ensures quality care and defensible documentation.

Payors and administrators demand the “golden thread” standard of integrated documentation. But, therapists weren’t given the tools to realistically achieve it — just more forms, more requirements, more time stolen from actual patient care.

The very documentation meant to improve care quality was making it harder to provide good care.

I knew this from my time in county clinics, and from reviewing the notes of other clinicians in our practice. I watched staff burn out, leave the field altogether, or stretch their documentation to serve administrators instead of the client. And still, clinicians faced clawbacks, reduced reimbursement rates, and audits.

That's what led me from the therapy chair to where I am today, leading clinical product development at Upheal.

  3. Review and refine: Edit the plan to reflect your clinical judgment. Add objectives, modify interventions, or adjust treatment areas as needed.
  4. Watch the magic: As you continue documenting sessions, your notes automatically reference relevant treatment goals, creating that golden thread of connection.
  5. Export and share: Copy your plan to your clipboard or download it as a PDF for your EHR or insurance submissions.
