Quality drift

Your tenth note rarely looks exactly like your first. Sometimes that is efficiency. Sometimes your documentation has stopped saying the important part. Your Assistant reviews a run of notes for patterns worth fixing.

Review my last ten notes and look for documentation drift. Flag repeated boilerplate, vague intervention language, unsupported progress claims, missing impairment or risk documentation, and places where my notes have gotten longer without getting more useful.

Example

Quality drift

Alex · Tue 2:00 PM · Session 14

Across the last 10 notes:

  • “Client was receptive” appears in 8 notes without behavioral detail.
  • Functional impairment is documented clearly in 3 of 10 notes.
  • Intervention sections have become longer, but response sections have become less specific.

Highest-value fix: replace generic response language with one observable client response per note.

Can do

This job only reads. It changes nothing.

It samples the habit, not one note

Look across multiple notes so recurring shortcuts and omissions are easier to see.

It finds the drift

Repeated weak spots, boilerplate, missing treatment links, and unnecessary verbosity get surfaced with examples.

You tighten the system

Fix the pattern once instead of rediscovering it note by note forever.

Your most popular FAQs

What does Quality drift look for?

Patterns across a run of your notes: sections that thinned out, phrasing that repeats from note to note, and important elements such as risk or response to intervention that stopped appearing. Efficiency is fine; missing the important part is not.

How many notes does it review?

A run you choose, such as the last month or the last twenty notes. More notes make the patterns clearer.

What do I get back?

A short checkup on your own documentation habits with examples, not a rewrite of individual notes. You decide what to change going forward.

Is this the same as the compliance check?

No. Note compliance check looks at one note against documentation standards. Quality drift looks across many of your notes for habits, including things a single note would pass on.
