What are SOAP notes in therapy?

SOAP notes are a standardized and concise way of capturing data from a client session including what the client is presenting with, their progress, and any future steps toward treatment.

SOAP notes are well-established in healthcare and are used by doctors, psychiatrists, nurses, therapists, psychologists, counselors, social workers, and more, to present a patient’s health findings in a clear and organized manner.

In mental health care, SOAP therapy notes help clinicians clearly document symptoms, progress, and treatment plans, and are increasingly created using AI SOAP note tools. Below is a SOAP note example from a member of our clinical team.

Here is an example from a real therapy scenario. How would you turn the following session into a mental health SOAP note?

Example: Treating a client who presents with major depression.

In this case, the contrast between both the physical presentation of depressive symptoms and self-reported emotional experiences will be important to tracking CBT effectiveness and the progress of treatment.

Here is a SOAP note example. It's been written by our Head of Clinical Operations, Ted Faneuff, a licensed therapist, to make sure it's truly helpful to you.

Subjective:

Client reports “I feel fine, I don’t understand why everyone keeps telling me to go to therapy.”

The client presents to therapy following the loss of their parent but states since they hadn’t talked to their parent in a long time, they are not feeling too bad about the loss.

The client stated that they have also struggled with a lack of self-care concerning their food intake, difficulty falling asleep and staying asleep, as well as loss of concentration at work.

Objective:

The client demonstrates flat affect, slow movements, infrequent eye contact, and dulled responses. Lack of self-care with poor hygiene and inability to focus is also noted.

The client struggled with attention during the session and exhibited slow response times in conversation.

Assessment:

The client appears to be exhibiting symptoms consistent with Major Depressive Disorder, as evidenced by reported sleep disturbances, poor self-care, concentration issues, and the observed flat affect and slow movements.

Plan:

The primary goal of therapy will be to reduce depressive symptoms. Cognitive Behavioral Therapy (CBT) will be employed to identify and challenge cognitive distortions contributing to the depression.

Homework will include tracking thoughts and behaviors related to depressive symptoms and practicing self-care routines. Progress will be monitored and interventions adjusted as necessary in future sessions.

Essential components of a SOAP note
